Alright, Here's my first opinion.
1. Biggest problem : damage
I like the arced look of the lightning, but its wholly unrealistic for its damage. The monster was one shotting me with full health and 200 armor. It offers little in the realm of counter play (especially when the monster can teleport right after you). The way rockets can really thrust the VIOS is weird too.
With that said, i do like the teleport, kinda cool.
2. Id suggest rather than having no blood (the nasty bullet puffs appear instead), it would be cool if you could find a cyan-like color for the blood that matches the color of the VIOS. not a big thing tho.
Other than that i have minor gripes about the choice of sound and some of the animations, but thats more of a personal opinion. Its decent-ish.

Well, without a full rotation set, its usage is very limited. I realize D2RPG didn't have full rotations for its monsters, but it wasn't a free-roaming shooter, and therefore didn't need the full suite of rotations like Doom does. We don't typically accept monsters with incomplete rotations, even if the incompleteness is due to the limitations of the original game's design.
The sprites are also a bit rough; they look like they've been upscaled from a somewhat lower resolution. The sprites were already 176x176 (if extracted from the iOS version), and shouldn't need any upscaling, particularly since they would already need to be significantly scaled-down in-game. Out of curiosity, how'd you acquire or extract the sprites, and what resolution were they in?
Some of the audio files are still in .WAV and therefore highly in need of compression. Some of them are already in .OGG, which is good.

I ripped them from phone screenshots. That's probably where the roughness comes from. I'll convert the sounds when I get back from school today.

I have the agree about the amount of damage being dealt. I think getting killed in one hit with 200 health and armor is too much.
Also, it would indeed be good to have some sprite rotations for it since it's more of a humanoid monster.

I thought that I fixed the damage issue. I'll check.
Doom 2 RPG didn't have rotations for VIOS.
EDIT: The lightning bolt doesn't one shot. Check the "now with OGG" link.
